Grindcore's got some messed up cover art. In particular:
Early Carcass
Pig Destroyer
Blood Duster
Anal Cunt
Really, any goregrind band, though grindcore's metal status varies.
And, from the death metal world, you've got Deicide, Cannibal Corpse, and of course, Devourment's "Molesting the Decapitated" wins every time.
As for thrash, Slayer's album covers could be considered messed up by some.
I should ask for rules about posting images. Should we simply link to offensive images, instead of posting them in their full, NSFW glory?

The inside art of Sepultura's "Chaos A.D." had some disturbing stuff in it, like the mutated woman and the sliced-up prisoners. Great album, though.
