Phantom Hourglass is a direct sequel to Wind Waker. Spirit Tracks is a timeline sequel. Link and Tetra find a new continent at the end of PH and name it Hyrule, and Spirit Tracks takes place 100 years later in New Hyrule.
The Oracle Games are sequels to each other if you want them to be. They play as stand alone adventures, but if you beat one, it gives you a password that you can put into the other and it changes the story a bit to make it a sequel, and the final boss becomes Ganon. Believe it or not, there were originally supposed to be three Oracle games, but it proved too difficult to implement the three games, two are sequels, play in any order system via passwords and the GBC. Supposedly, one game would correspond to each piece of the Triforce. Also, Miyamoto wanted part of the plot of the games to be that there was originally a fourth piece of the Triforce (the missing middle), but someone in the distant past ate it. Take that as you will.
No one gives a shit about Four Swords.

Wind Waker and Phantom Hourglass are the same Link. They take place within one year of one another. Spirit Tracks is a century after Phantom Hourglass with a new Link, but Nico the pirate is still kicking around and he's old as heck, and he talks about the WW/PH Link.
Oracle games take place one immediate after another in either order.

Near as I can figure that's Nintendo saying "What if Link failed in OoT? This timeline is what would result." I've played every one of the canon Zelda titles (no CD-i sadly) and the death of the hero is never referenced...
Yeah it's a bit of a cop-out on their part, but at least their explanation makes sense within the mythology of the games that follow that branch. The question really will be is if future games in the Hero Defeated timeline will make reference to OoT's Link getting a sword in the gut.

That's tricky. If you read the Zelda II instruction manual, it says that long ago when Hyrule was a single country (so post-unification war), the king maintained order using the Triforce. His son only inherited part of the triforce and quested to find the other pieces. Some magician told the prince that his sister knew something about it, but she wouldn't tell him. The magician put her into an eternal slumber despite the prince's protests. The magician was slain but it was too late to save the princess. The prince orders all future daughters of the royal line to be named Zelda.
Of course, this doesn't ever appear in any game. So it could just be a thing between games.
Also, Skyward Sword totally messed with the "all female children shall be named Zelda" thing by adding another layer to it...

My two cents:
When Link goes back into the past to go into the well or Spirit Temple, he creates a new timeline. He leaves the old timeline, which now has no Link in it, so there is no one to stand up to Ganon. Essentially, not having the tools to proceed, Link must abandon that timeline to destruction while he journeys to the past to arm himself.

For a moment I thought the Skyward Sword Zelda was going to be that Zelda. It would have been cool if they did that but of course they didn't because it would resemble some shred of continuity.

So Hyrule Historia has confirmed that the Hero's Shade from Twilight Princess is the Hero of Time (Link from OoT/MM). So that makes three consecutive games for that particular Link.
Also, that means we have a canon instance of Link speaking. Whoh.
